This service supports people  who have an intellectual disability  to live in their own homes within the general community.  The service is provided by New Horizons in Taree and Port Macquarie.

The target group is:

• People aged 18 – 65 years

• People with an intellectual disability

The service will:

• Facilitate social and community networks

• Provide case coordination

• Link clients to supportive health professionals

• Provide activity and training opportunities

• Facilitate planning of independent living skills
