The New Horizons Transitional Support Service in Northern Sydney provides comprehensive support to service recipients during their period of transition from custody back into the community. This program enables them to establish appropriate support networks and life skills in the community.
The target group is people who:
• Are over the age of 18
• Are male
• Have a mental illness and/or mental disorder
• Are to be released from a correctional centre
• Have recently been released from a correctional centre
• Are committed to employment
The services provided are:
• Case Management
• Training in job seeking skills
• Links with employment services
• Securing long term accommodation
• Linking with educational facilities
• Linking to Government services
• Connecting with family
• Connecting with other support networks
• Collaboration in the development and implementation of an Individual Personal Plan
• Counselling and education
• Medication management and training
• Training with daily living skills
• Promote and encourage decision making and independence
• Future planning
• Advocacy
