New Horizons manages short term Community Support Services which include Case Management, Brokerage and Outreach Support Services to people with a disability who are residing in the North Sydney, Mosman, Lane Cove, Ryde, Manly, Willoughby, Warringah, Pittwater, Hunters Hill, Hornsby and Ku-ring-gai Local Government areas.

New Horizons outreach services have been developed in line with the Disability Services Standards. They focus on linking individuals to a range of home based resources that enable them to remain in the community.

The target group is people with:

• an intellectual disability
• an intellectual disability and a mental illness
• an acquired brain injury
• a physical disability

• Dual Diagnosis

The services provided are:

• Service co-ordination and planning
• Linking individuals and family to appropriate services
• Life skills training
• Travel training
• Vocational counselling
• Social/recreational activities
• Educational training
• Respite care
• Therapy
• Family support
• Government agencies and industry networks
