New Horizons YPIRAC provides a case management service to younger people less than 65 years living in residential aged care facilities. We cover Sydney, Central Coast, Hunter, Mid North Coast and Far North Coast. At this time priority will be given to people less than 50 years. The focus of YPIRAC is to improve the life outcomes of these younger people who typically are living with frail aged persons who may have dementia. YPIRAC is mostly free to clients because it is funded by DADHC.

The Target group is people with:

- A neurological illness such as MS

- An acquired brain injury

- A physical injury

- An intellectual disability

- A psychiatric disability

The Services provided are:

- Small group activities

- Individual activities and outings

- Allied health

- Support to visit family and friends

- Transport to community and services
