Hospital to Community Transition Service (HCTS) provides clients with intensive, non-clinical disability support for up to six weeks, following a planned discharge from hospital.
Clients returning to their own home with appropriate support receive assistance to strengthen or maintain the gains made in hospital, re-establish routine activities
and reconnect to social supports.
This service aims to address the housing needs of clients and, in turn, free up hospital beds by assisting clients to overcome common barriers such as a lack of social/family support at home, isolation from the community, or the need for assistance with household activities such as shopping, cooking and cleaning.
By providing an earlier discharge option and support at home, access to inpatient services and the rate of readmission may also be improved.
HCTS is delivered in partnership with NSW Health's Northern Sydney & Central Coast Mental Health, Drug and Alcohol (NSCCMHDA).
If you are over the age of 16, a current client of NSCCMHAD and have a primary diagnosis of a mental illness or psychiatric condition this service may be available to you.
We also ask that you have permanent accommodation and have a willingness to undertake HTCTS support.
New Horizons currently provides the HCTS in Sydney, from our office at North Ryde, and on the NSW Central Coast from our office at Gosford.
Referrals are made by NSCCMHDA and forwarded to the appropriate New Horizon’s contacts listed on the right of this page.
