Life Choices

Life Choices Self Managed Model is a day program which aims to provide adults with disability a broader range of options, increased choice, and greater flexibility for their support needs.

As part of the program, clients choose the things they want to do, the services they want to access, and the way they want to use their Life Choices funding.

We assist clients to achieve their goals by developing a person-centred, individual plan, arranging activities and managing all of the financial, legal and administrative requirements of the Life Choices Program.

  • Create opportunities and pathways to achieve your goals
  • Make friends with people who share similar interests
  • We will listen to you, learn what you want to do and provide support to you to:
  • Decide what you want to achieve and set goals
  • Participate in activities and develop skills
  • Get the most out of your program and individual funding

Who can access Life Choices?

Life Choices Self Managed Model is open to people with a disability between the ages of 25 and 54. Referrals are made through NSW Ageing, Disability & Home Care (ADHC).
