New Horizons manages short term Community Support Services which include Case Management, Brokerage and Outreach Support Services to people with a disability who are residing in the North Sydney, Mosman, Lane Cove, Ryde, Manly, Willoughby, Warringah, Pittwater, Hunters Hill, Hornsby and Ku-ring-gai Local Government areas.
Our Outreach Services have been developed in line with the Disability Services Standards. They focus on linking individuals to a range of home based resources that enable them to remain in the community.
