Personal Helpers & Mentors

Personal Helpers & Mentors (PHaMs) provide direct, personalised outreach support to assist people whose ability to live independently is impacted by mental illness.

The service aims to increase opportunities for recovery, and provide the support necessary to achieve improved quality of life.

Personal helpers & mentors work one- on-one to create Individual Recovery Plans that focus on goals and strengths, and assist people to better manage their daily activities, access appropriate support, reconnect with family & friends, achieve independence and reach personal goals.

How we can support you

Peer support, advocacy, mediation, referral, access to services, training and support. These are just some of the many ways personal helpers and mentors can support your recovery journey.

We will support you to:

  • reconnect with your community
  • access services that meet your needs
  • engage in the activities you enjoy
  • recognise your strengths
  • create a plan for your recovery
  • achieve your personal goals
